Informed Consent & Health Disclosure

This is an agreement between you and your Artist, presented before your appointment. Charon is not a party to it — see Charon's Terms for what Charon is and isn't responsible for.
1. What You're Agreeing To
You are requesting a tattoo from the Artist named on your booking. Before that appointment happens, you're confirming that you understand what a tattoo involves, that you've disclosed anything about your health that could matter, and that you're making this decision for yourself, without pressure, coercion, or impairment.
2. Understanding the Procedure
A tattoo is created by repeatedly puncturing the skin with a needle to deposit ink into the dermis. This is:
- Permanent. Tattoos do not wash off or fully fade. Removal, where possible at all, is a separate, often costly and imperfect process — not something Charon or your Artist provides or guarantees.
- Imprecise by nature. Skin, ink, and healing all vary from person to person. Color, line sharpness, and how a tattoo settles over the following weeks cannot be guaranteed exactly as previewed or discussed beforehand.
- Carries real physical risk, described in the next section, even when your Artist follows proper sterile technique.
3. Risks You're Acknowledging
You understand that possible risks of tattooing include, but are not limited to: infection; allergic reaction to ink, latex, or other materials used; scarring or keloid formation; uneven healing, fading, or blowout; skin sensitivity or irritation, including reactions triggered by sun exposure later; and, in rare cases even with sterile single-use needles and proper technique, exposure to bloodborne pathogens. You understand these risks exist regardless of your Artist's skill or care, and that no outcome is guaranteed.
4. Health Screening
You confirm the following, to the best of your knowledge:
- You are not currently pregnant or breastfeeding.
- You do not have a condition that impairs blood clotting, or are not currently taking a blood-thinning medication, without having discussed it with a physician first.
- You do not have a skin condition (such as active eczema, psoriasis, or a skin infection) at or near the intended tattoo site.
- You are not currently under the influence of alcohol or any drug that impairs judgment or blood clotting.
- You have disclosed any known allergies — including to latex, adhesives, or ink pigments — to your Artist before this appointment.
- You have disclosed any other medical condition your Artist would reasonably need to know about to perform this service safely.

5. Photographs and Placement
You confirm you have reviewed and approved the final design and placement with your Artist before the tattoo begins, and that any reference images or inspiration you provided are not being used to reproduce another artist's copyrighted work without your Artist's independent creative judgment.
6. Signature
By checking the box presented with this document, you confirm you have read and understood it, that everything you've disclosed is accurate, and that you consent to the tattoo procedure described in your booking. Charon records the exact version of this document, the time, and your account at the moment you agree, and keeps that record on file.
